Permission to photograph/video your child(ren) for church VBS slideshow and/or online promotional use

I give permission for my child(ren) to participate in the Wonder Junction VBS Program, games, and activities under the supervision of the VBS Leaders and Game Directors. I understand that my child(ren) is expected to follow the Code of Conduct of Pleasant Hill Baptist Church of Gainesville and to behave appropriately. I release and waive, and further agree to indemnify, hold harmless or reimburse Pleasant Hill Baptist Church of Gainesville, the individual members, drivers, employees, and representatives thereof, as well as trip supervisors, from and against, any claim which I, and other parent or guardian, any sibling, the student unknown, directly or indirectly for any losses, damages, or injuries arising out of, or in connection with the child’s participation in any church VBS activity, or trip, or the rendering of emergency medical procedures or treatment, if any.
